Visa Application Forms
All applicants must present the following form at the interview:
DS-156

- This form must be filled out on-line and then printed. The DS-156 may not be submitted electronically.
- You must answer all questions. Failure to do so may result in the cancellation of your interview or the refusal of your application.
- Your printed form should consist of three pages. The bar code on the third page must be complete and printed clearly. Our computer system will not accept your application if the bar code is broken or incomplete.
- You must sign and date your application. If someone else completes the application on your behalf, they must sign and date as well. The applicant remains responsible for the accuracy of the information on the form.
Male applicants over the age of 14 must also present the following form:

- You must answer all questions. Failure to do so may adversely affect your application.
- You must list all countries you have entered in the last ten years (per question #9). Failure to provide complete information can result in lengthy delays in visa processing. You are welcome to attach an additional sheet of paper to answer this question. We recommend including the date, duration, and purpose of each trip.
- This form may be completed on the computer or by hand.
Additional Visa Forms for Certain Categories of Applicants
Students and Exchange Program Participants
(Visa categories F, J, and M)

- This form requests additional information about you, people we can contact in an emergency, and your work experience.
- This form may be completed on the computer or by hand.
I-20 or DS-2019
- The college or university in which you will study, or the organization arranging your exchange program will provide this form. You must present the original form to the Embassy. The Embassy will return the original form to you after processing your visa application.
I-901 SEVIS Fee Receipt
- All F, J, or M visa categories must pay an initial fee to register with the Student and Exchange Visitor Information System (SEVIS). Applicants may pay this fee online or at Western Union.
